Job Summary
Manage geriatric patient cases by applying case management processes and coordinating with healthcare professionals. Utilize knowledge of funding resources including Medicare Parts A, B, C, D, Medicaid, and private insurances to support client needs. Address crises and demonstrate creative problem-solving within the NHC HomeCare Hendersonville team. Required Qualifications
- Master's degree in social work from an accredited program
- knowledge of the case management process
- needs of geriatric patients
- working knowledge of funding resources such as Medicare Part A, B, C and D, Medicaid, Managed Care and other private insurances
