Part Time Licensed Practical Nurse

Licensed Practical Nurse performs home-based nursing assessments and procedures under the guidance of the RN Supervisor, implementing the plan developed by an RN in accordance with physician orders. Responsibilities include tracheostomy and colostomy care, catheterizations, wound/dressing changes, phlebotomy, monitoring vital signs and intake/output, patient and family teaching, maintaining patient confidentiality, coordinating with the health-care team, and ensuring accurate documentation under Medicare/Medicaid/HIPAA regulations. Requires graduation from an accredited school of practical nursing, state licensure as an LPN/LVN, and at least two years of clinical nursing experience; domicile-based home visits with reliable transportation. Flexibility to work a 3pm-11pm schedule and participate in a team delivering patient-centered home care.
