Part Time Hospice Registered Nurse or Licensed Practical Nurse

Job Summary
Provide one-on-one home hospice care as an RN or LPN, including supervision of paraprofessionals, assessments, and development/revision of the patient/family plan of care. Conduct initial and ongoing assessments, ensure Medicare/compliance adherence, educate patients/families on disease processes and end-of-life care, participate in IDG conferences, and support on-call rotations. Must hold an active Michigan nursing license and have hospice experience; role involves field visits and Đ³Đ¸Đ± flexible scheduling 8-16 hours per week with on-call responsibilities.
Required Qualifications
- Current Michigan License in good standing
- Minimum 1 year hospice experience
- Empathetic approach to patients and caregivers
- Participate in field visits daily, weekly office meetings as indicated
- Ability to adapt to patient’s status and needs on a full time on call basis
