Part-Time Hospice Certified Nursing Assistant/AIDE in Longview, TX. Work with the RN Case Manager to provide best in class hospice care to a reasonably sized patient census, presenting at IDT every 2 weeks with updates from the care team, and coordinating with the RN Case Manager, Clinical Director and Medical Director. Must be a certified Nursing Assistant with a current license and keep continuing education current. Responsibilities include adhering to regulatory standards, following professional standards, identifying system problems affecting patient care, managing clinical excellence in patient census, maintaining high levels of internal and external communication, and collaborating to deliver a concierge hospice experience.
