Part Time Hospice and Palliative Nurse Practitioner

Hospice and Palliative Nurse Practitioner (Part Time) provides and documents medically necessary hospice/palliative care services, participates in care planning and decision-making for recertification and discharge, visits patients at home to assess recertification suitability and make treatment recommendations, develops and implements treatment plans, orders and interprets tests, and collaborates with interdisciplinary teams to ensure quality care. Responsibilities include collaboration with physicians and other specialists, coordination of referrals and reimbursement processes, development of outreach programs with Marketing, patient/family education, guidance to facility staff, on-call responsibilities, and maintenance of accurate medical records in compliance with HIPAA.
