Job Summary
Perform comprehensive physical therapy evaluations, develop individualized treatment plans, and provide skilled interventions to improve patient mobility and strength. Educate patients and caregivers on home exercise programs, monitor progress, adjust care as needed, and document sessions accurately. Collaborate with physicians and the interdisciplinary team to ensure coordinated, high-quality patient outcomes. Maintain productivity standards while delivering compassionate, one-on-one care in a home health setting. Monday–Friday schedule from 8am to 5pm with no weekend requirement.
Required Qualifications
- Minimum of 9 months of experience as a Physical Therapist
- Master's degree in Physical Therapy from a CAPTE-accredited physical therapy program
- Current Physical Therapist license in the state of practice
- Current CPR certification
- Valid driver's license
- Reliable transportation
- Ability to travel throughout the assigned service area
