Job Summary
Assist patients with daily living activities, grooming, and comfort while maintaining safety. Work as part of the care team to help patients reach their goals and attend weekly case conferences to coordinate care with the case manager and team. Requires daily Monday-Friday availability starting at 8 am for 15-20 hours per week. An active CNA license, valid driver's license, and reliable transportation are mandatory.
