Job Summary
Provides personal and hygienic care per care plan and basic care standards within the patient’s home setting under RN supervision; follows plan of care, relays critical information to RNs, documents patient status, and collaborates with the health care team. Duties include measuring vital signs, transferring/ambulating patients, ROM, bathing/personal hygiene, and assistance with activities of daily living, while adhering to safety and policy guidelines and ensuring cost containment. Primarily for Randolph and Pocahontas Counties; positions involve home visits in patients' homes with travel and exposure to healthcare environments.
Required Qualifications
- High school diploma or equivalent
- Certification issued by state where services will be performed (CNA/Nurse's Aide as appropriate by state: MD, OH, PA, WV)
- Valid driver’s license
- CPR Certification within 30 days of hire
