Job Summary
Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, toileting, and meal preparation. Provide medication assistance, housekeeping, laundry, shopping, transportation, and companionship as specified in the client's plan of care. Report daily activity through the agency management system and activity logs, maintaining confidentiality and adhering to HIPAA regulations. Practice safety in all activities, remain vigilant of potential care concerns, and report any suspected abuse or neglect immediately. Document out-of-the-ordinary behaviors, events, or conditions while ensuring personal appearance complies with company policies. Maintain a valid driver's license, reliable transportation, and "Fit for Duty" physical condition to perform essential functions.
Required Qualifications
- Must possess required skills and be able to explain and demonstrate, with or without reasonable accommodations, that the essential functions of the job can be performed
- Maintain an established work schedule
- Maintain confidentiality of work-related information and materials
- Maintain 'Fit for Duty' physical condition
- Have a valid driver's license
- Have automobile insurance
- Be 18 Years of Age or Older
- Have reliable transportation
- Keep your personal appearance in compliance with Avenues Home Care Policies, including abiding by company dress code
- Be timely in reporting for shifts or making scheduling changes
- Report any suspected abuse or neglect immediately up the proper chain of command, as required by law
- Abide by all policies and procedures set forth by the Company
- Ensure protection of confidential information that is stored, handled, or maintained by following Company policies and procedures related to confidentiality and/or HIPAA regulations
- Work safely and follow safety rules
- Report unsafe working conditions and behavior
- Be required to lift or push up to 50 pounds
- Demonstrate honesty, accountability and integrity
- Provide superior care to Avenues Home Care clients
- Remain vigilant of any potential care concerns or change in condition
- Work requires good communication skills
- Ability to follow verbal and written assignments
- Demonstrate proficiency with Homecare Company Management System--ClearCare
Desired Qualifications
- CNA License or alternate experience as a caregiver
- CPR & First Aid Certifications
- Prior experience assisting with ADLs or caring for elderly, disabled and other vulnerable clients
