Job Summary
Conduct initial client assessments and ongoing supervisory visits throughout the Hampton Roads area. Assess client needs to develop or update individualized care plans, then complete required documentation and communicate with staff regarding service authorizations. Monitor client progress, provide guidance and training to Personal Care Aides, and verify their skills and competencies. Travel to clients' homes to assist with personal care needs when necessary. Requires a valid Virginia nursing license, reliable transportation, and ability to work independently on a flexible part-time schedule. Mileage reimbursement is provided for work-related travel.
Required Qualifications
- Valid nursing license in the Commonwealth of Virginia
- Valid driver's license
- Reliable transportation
- Ability and willingness to travel to client homes throughout the service area

- Home care or similar healthcare experience
- Comfortable visiting homes that may have pets or smoking
