Accompany residents to appointments, dining, errands, community outings, and social activities. Coordinate and provide transportation as needed. Create personalized engagement opportunities that enhance quality of life. Communicate resident updates to the Care Navigation team and maintain accurate documentation. Assist with appointment scheduling, move management, and other resident support services. Collaborate with residents, families, and team members to deliver an exceptional experience.
