Job Summary
RN Care Manager with in-home nursing responsibilities and supervisory duties. Role includes oversight of in-home caregivers, routine supervisory visits per state regulations, mentorship of caregivers, and education on clinical issues. Responsibilities include clinical leadership, coordinating with the Interdisciplinary Team (IDT), and expanding patient census through outreach to local healthcare professionals and referral sources. Hands-on tasks may include wound care, medication administration and box setup, catheterizations, blood draws, intake assessments, and care management services. Requires the ability to perform in-home nursing tasks (wound care, meds, dressing changes) and to operate within a team-driven care model across multiple offices nationwide. Educational emphasis is on RN practice with an ADN (preferred BSN/MSN) and strong communication with clients, families, physicians, and care teams.
Required Qualifications
- Associate Degree in Nursing (ADN) required
- Active RN license in the applying state
- Minimum of two years of experience in geriatrics, home care, home health, and/or care management
- Ability to work in a team environment
- Excellent written and verbal communication skills
- PC literacy
- Valid driver’s license and proof of auto insurance
