Job Summary
Care Coordinator role in Crossroads Treatment Centers' outpatient MAT program in Oak Ridge, TN. Responsibilities include meeting face-to-face with patients to facilitate recovery services and document activities in the patient record, working with collateral contacts and local referral agencies to provide services not offered by the program, conducting monthly Care Coordination meetings to ensure patient engagement, and collaborating with buprenorphine- or naltrexone-prescribing clinicians for individuals with moderate to severe opioid use disorder. Requires a bachelor’s degree in a relevant field or RN/CSAC credentials with applicable clinical experience; scheduling includes Tue 9:00am-2:00pm, Wed 9:00am-5:00pm, Thu 12:00pm-5:00pm; includes benefits and a supportive organizational culture focused on recovery and equity.
Required Qualifications
- A bachelor’s degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling and at least either one year of experience providing services to individuals with a diagnosis of substance use disorders or a minimum of one year of clinical experience working with individuals with cooccurring diagnoses of substance use disorders and mental illness.
- Licensure as a Registered Nurse with at least either one year of direct experience providing services to individuals diagnosed with a substance use disorder or a minimum of one year of clinical experience working with individuals with cooccurring diagnoses of substance use disorders and mental illness.
- Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC-Assistant under supervision of a CSAC or another supervisor as defined in 18 VAC 115-50-10
- Schedule Tuesday; 9:00am-2:00pm Wednesday; 9:00am-5:00pm Thursday; 12:00am-5:00pm
