Job Summary
Provide medical care to a complex patient population in short-term rehab and long-term care settings by analyzing medical history, ordering diagnostic tests, and prescribing treatments. Coordinate with nursing facility care planning teams to develop comprehensive care plans and communicate with patients and families regarding admission, discharge needs, and prognosis. Record physical findings and formulated plans within appropriate time frames while following departmental policies and safety standards. Travel to Skilled Nursing Facilities contracted with Physicians Alliance of Connecticut (PACT, LLC) in the New London area. Requires a valid APRN or PA license, national certification, and experience with elderly populations and EHR systems.
Required Qualifications
- Proof of annual flu vaccination
- Masters or doctorate prepared
- Certified Nationally as a Nurse Practitioner through AANP or ANCC
- Valid (Connecticut) license as a Registered Nurse
- Advanced Practice Registered Nurse certification
- DEA license
- Ct controlled substance license
- Experience working with elderly patient population
- Experience working with EHR
- Physician Assistant Certification
- State License (for PA)
