Job Summary
Facilitate access to mental health programs and housing for clients and families by assessing needs, completing applications, and providing culturally sensitive referrals. Provide field-based, trauma-informed support services directly in client homes, schools, or other locations while working as part of a dedicated multidisciplinary team. Assist families in improving daily living skills and follow up on case management needs through appropriate linkages and consultations. Maintain 24/7 on-call availability by phone and attend all required meetings, supervision, and case reviews as assigned. Ensure all documentation and service time requirements are met while traveling throughout Los Angeles County.
Required Qualifications
- GED or High School Diploma
- Experience as a parent or care-providing family member of a consumer of mental health services, DCFS, or Department of Probation services
- Spanish fluency
- Valid California driver's license
- Insurable driving record under the Clinics' liability policy
- Class II California driver's license (may be required)
- Pass all applicable health, fingerprint/LiveScan and DMV clearances
- Must be able to provide all services in the field including client homes, schools or other locations
- Must be able to lift and/or move up to 10 pounds
