Job Summary
Facilitate Parent Engagement Meetings at elementary schools to define attendance barriers, identify family strengths and needs, and develop action plans preventing unnecessary child welfare involvement. Conduct home visits within five working days of referrals, assess families using the NCFAS and ASQ-3, and collaborate to develop written goals and safety plans. Schedule program planning meetings, establish shared calendars, and enter meeting data into the GenTrack reporting system. Provide follow-up calls to parents within three to four weeks and attempt six-month check-ins to monitor family wellbeing. Maintain confidentiality by securing case files and adhering to HIPAA standards while advocating for the best interest of the child.
Required Qualifications
- Ability to lift from the floor to 18" above the shoulder
- Vision sufficiently correctable to permit full performance of job duties
- Ability to drive for travel within the service area and beyond, if necessary
- Wide range of mobility
- Able to perform clerical functions such as filing, labeling, stamping, and writing
- Must be able to lift/manipulate/move objects up to 50 lbs
- Must hold a bachelor's degree in Family or Human Services or a related field OR hold a bachelor's degree in an area outside of human services but have at least one year of experience in providing in-home services
- Must possess and maintain a valid driver's license
- Drug screen and background check required
- All employees are required to maintain the confidentiality of private information
- All employees must strictly adhere to HIPAA standards that govern the use of sensitive and confidential information
