PARENT ADVOCATE/CASE AIDE

Job Summary
Open and close case records, send required notices, and document activities in tracking logs, PROMIS, CONNECTIONS, and Excel. Complete monthly chart reviews of at least 10 case records and compile data on client demographics and service utilization to inform program evaluation. Answer phones, greet individuals and families to assigned workers, and coordinate trainings for all program staff. Support parent advocates with case assignments, conduct interviews to obtain collateral documents, and assist families in connecting to community resources and improving parenting skills. Participate in initial intakes, contacts, and service plan meetings while maintaining strict confidentiality. Schedule and attend monthly unit, interdisciplinary, and clinical team meetings. Work hours vary based on program needs.
Required Qualifications
- HS Diploma or GED
- Child welfare experience
- Computer skills including Microsoft Word, PowerPoint, SharePoint, Teams
- Ability to learn other systems
