Paraprofessional

Paraprofessional Assistant Coordinators provide in-home social services for at-risk children and families, developing personalized service plans, scheduling weekly visits, and ensuring safety and well-being per APS Healthcare Utilization guidelines. Responsibilities include opening cases within mandated timeframes, documenting interventions and client responses, coordinating with DHHR case managers, maintaining client records, participating in multidisciplinary team meetings, testifying in court when needed, attending staff trainings, and delivering supportive services such as Respite and Independent Living/Skills Building. Requirements include a High School Diploma or GED, valid driver's license and insurance, ability to pass a fingerprint background check and APS/CPS screening, effective communication with families and professionals, and proficiency in Microsoft Office and general computer skills.
