Job Summary
Provide ethical and safe nursing care within the community setting for palliative patients, performing scheduled visits, executing and monitoring care plans, and maintaining patient records. Collaborate with RNs for complex cases, build therapeutic relationships with patients and families, and participate in a comprehensive orientation and ongoing training. Requires CNO registration as an RPN, a valid driver’s license, reliable transportation, and willingness to travel within the designated area; on-call coverage is required to ensure patient safety.
Required Qualifications
- Registration with the CNO as a Registered Practical Nurse in good standing
- Palliative care experience mandatory
- Valid driver’s license and access to a vehicle
- Experience in delivering community care is an asset
- Willingness to travel to and from patient residences within a designated area
- On-call coverage, including after hours
