Job Summary
Provide comprehensive palliative care assessments and symptom management to individuals facing serious illness in the comfort of their homes and care facilities. Support patients and families through goals-of-care discussions and advanced care planning while collaborating with physicians, nurses, social workers, and interdisciplinary team members. Manage complex and chronic conditions with a focus on comfort and quality of life, educating patients, caregivers, and staff on palliative care services. This role offers autonomy and flexibility to manage your own schedule, with no holidays, weekends, or on-calls. Licensed and board certified Nurse Practitioners in Pennsylvania are required. Concordia Visiting Nurses delivers high-quality skilled nursing and therapy services to patients in Western Pennsylvania.
Required Qualifications
- Licensed and board certified as a Nurse Practitioner in Pennsylvania
- Experienced in palliative care, hospice, home health, geriatrics, or chronic disease management
