Job Summary
Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms for patients with complex, chronic, or serious illnesses. Complete face-to-face encounters for hospice patients as needed and develop individualized care plans in collaboration with the interdisciplinary team. Prescribe medications and coordinate care while accurately documenting all encounters in the EMR. Communicate effectively with patients, families, and other healthcare professionals. Active Wisconsin Nurse Practitioner license required; experience with eClinicalWorks preferred. Part-time (20 hours/week) or full-time (32–40 hours/week) opportunities available.
Required Qualifications
- Active Nurse Practitioner license in Wisconsin
Desired Qualifications
- Experience preferred but willing to train new Nurse Practitioners!
- Strong clinical assessment, documentation, and care planning skills preferred
- Experience with eClinicalWorks (eCW) EMR preferred (willing to train)
- Excellent communication and teamwork abilities
