Job Summary
Palliative Care Nurse Practitioner (part-time) in Eau Claire, WI and surrounding areas. Conduct comprehensive palliative care visits, assess, diagnose, and manage symptoms; complete F2F encounters for hospice patients; develop and implement individualized care plans with an interdisciplinary team; prescribe medications and coordinate care for patients with advanced or chronic illnesses; document encounters in the EMR; communicate effectively with patients, families, and other healthcare professionals. Prioritized 1:1 clinician-to-patient ratios, flexible assignments, EMR training, mileage reimbursement, and opportunities for CEUs and tuition discounts. Active Wisconsin NP license required; strong clinical and teamwork skills desired; experience with eClinicalWorks EMR preferred. Part-time role with remote not advertised; on-site in Eau Claire, WI.
Required Qualifications
- Active Nurse Practitioner license in Wisconsin
- Experience preferred but willing to train new Nurse Practitioners
- Strong clinical assessment, documentation, and care planning skills preferred
- Experience with eClinicalWorks (eCW) EMR preferred (willing to train)
