Job Summary
Provide direct patient care under the supervision of a palliative care registered nurse, implementing treatments and managing pain and symptoms while evaluating patient responses. Follow the interdisciplinary plan of care, conduct comprehensive assessments, and provide accurate documentation of all services. Educate patients and families on diagnosis and self-care strategies, and coordinate palliative services by reporting identified needs to the interdisciplinary group. Use equipment and supplies effectively, participate in quality improvement teams, and perform other duties as assigned. Requires current LPN/LVN licensure, three years of nursing experience, and CPR certification.
Required Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Three years nursing experience
- Currently licensed as an LPN/LVN in the State
- Complies with accepted professional standards and practice
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
- Possesses and maintains current CPR certification as required by organizational policy
Desired Qualifications
- Community health/hospice or medical/surgical and palliative care experience
- Experience and/or training in the developmental stages and needs of infants, children and adolescents
- Demonstrates good verbal and written communication, and organization skills
- Excellent observation, verbal and written communication skill, problem solving skills, mathematical skills; nursing skills per competency checklist
- Understands philosophy of palliative. Also understands needs of the terminally ill
