Job Summary
Provide biopsychosocial assessments and develop, implement, and monitor individualized care plans for patients in hospice/palliative care; deliver counseling and education to patients, families, and caregivers; assist with advance care planning including Durable Power of Attorney (DPOA-HC); deliver staff training and contribute to curriculum and resources (The Reading Room); collaborate with interdisciplinary teams, participate in care conferences, ensure timely documentation and compliance with Medicare Hospice Conditions of Participation and Illinois Social Work Practice Act; this is a full-time, 2-year grant-funded community-based role requiring travel in the Rockford, IL area.
Required Qualifications
- MSW from a CSWE-accredited program
- Active Illinois Social Worker license
- 1+ year experience in healthcare, hospice, or palliative care
- Valid driver’s license, vehicle, and reliable transportation
