Palliative Care Registered Nurse (RN)

Job Summary
Coordinate care for assigned patients and families by completing comprehensive physical and psychosocial assessments to identify intervention needs. Ensure appropriate specialist referrals, support adherence to treatment plans, and assist in accessing community resources to improve overall health. Educate patients on disease processes and treatment options to facilitate informed decision-making while providing emotional support and counseling. Evaluate recovery progress, empower patients through goal-setting and advance care planning discussions, and partner with clinicians to manage transitions between hospital, primary, and specialty care. Facilitate proactive discharge planning to promote better outcomes and reduced readmissions. Protect patient privacy and confidentiality. Requires a Bachelor's degree, current RN license, BLS certification, and 2+ years of nursing experience.
Required Qualifications
- Minimum 2 years' experience as Registered Nurse
- Current and Valid Registered Nurse License in the state position is based
- BLS certification required
Desired Qualifications
- 1–2 years of nursing experience in a community-based setting, such as home health, rehabilitation, hospice, or palliative care, preferred
- Knowledge of the interdisciplinary team concept
- Experience in patient education, planning, and management desired
- Exceptional communication and customer service skills
- Reliable transportation, current state driver's license and automobile insurance
- Bilingual a plus
- Bachelor's degree preferred
