Palliative Care Registered Nurse (RN)

Coordinate care for assigned patients and provide expert, individualized support to patients and families. Complete initial and ongoing comprehensive physical and psychosocial assessments to identify needs and areas for intervention. Ensure appropriate referrals to specialists, support adherence to treatment plans, and assist patients in accessing community services. Educate patients and families on disease processes and treatment options to ensure informed decision-making. Provide emotional support and counseling to improve transitions and adherence to care plans. Build relationships with clinicians, partner with healthcare teams to manage care transitions, and facilitate proactive discharge planning. Protect patient privacy and confidentiality when releasing information. This role is part of VITAS Healthcare's growing palliative care program, focusing on individualized, coordinated care for patients and families in the community.
