Job Summary
Coordinate care for assigned patients by completing comprehensive physical and psychosocial assessments to identify intervention needs. Ensure appropriate specialist referrals and support adherence to treatment plans, including physician visits. Educate patients and families on disease processes and treatment options to facilitate informed decision-making. Provide emotional support and counseling to improve transitions and adherence to care plans. Empower patients through goal-setting and advance care planning discussions while building relationships with the broader healthcare team. Facilitate proactive discharge planning and follow-up to promote better outcomes and reduced readmissions. Protect patient privacy and confidentiality when releasing information.
Required Qualifications
- Minimum 2 years' experience as Registered Nurse
- Current and Valid Registered Nurse License in the state position is based
- BLS certification required
Desired Qualifications
- 1–2 years of nursing experience in a community-based setting, such as home health, rehabilitation, hospice, or palliative care, preferred
- Knowledge of the interdisciplinary team concept
- Experience in patient education, planning, and management desired
- Exceptional communication and customer service skills
- Reliable transportation, current state driver's license and automobile insurance
- Bilingual a plus
- Bachelor's degree preferred
