Palliative Care Registered Nurse (RN)

Job Summary
Coordinate care for assigned patients and provide expert, individualized support to patients and families residing in the community. Complete initial and ongoing comprehensive physical and psychosocial assessments to identify needs and areas for intervention. Ensure appropriate referrals to specialists, support adherence to treatment plans, and assist patients in accessing community services to improve overall health. Educate patient and family on disease processes and treatment options to ensure informed decision-making while providing emotional support and counseling. Evaluate physical and emotional recovery, empower patients through goal-setting and advance care planning discussions, and partner with clinicians to manage transitions between hospital, primary, and specialty care. Facilitate proactive discharge planning to promote better outcomes and reduced readmission rates.
Required Qualifications
- Minimum 2 years' experience as Registered Nurse
- Current and Valid Registered Nurse License in the state position is based
- BLS certification required
Desired Qualifications
- 1–2 years of nursing experience in a community-based setting, such as home health, rehabilitation, hospice, or palliative care, preferred
- Knowledge of the interdisciplinary team concept
- Experience in patient education, planning, and management desired
- Exceptional communication and customer service skills
- Reliable transportation, current state driver's license and automobile insurance
- Bilingual a plus
- Bachelor's degree preferred
