Palliative Care Registered Nurse (RN)

Coordinate care for assigned patients by completing physical and psychosocial assessments to identify intervention needs. Ensure appropriate specialist referrals, support adherence to treatment plans, and assist patients in accessing community resources. Educate patients and families on disease processes and treatment options to facilitate informed decision-making. Provide emotional support and counseling to improve transitions and adherence while empowering patients through goal-setting and advance care planning. Partner with clinicians to manage care transitions between hospital and specialty settings, facilitating discharge planning to reduce readmissions. Protect patient privacy and confidentiality when releasing information. Minimum two years of nursing experience required, with a Bachelor's degree preferred.
