Job Summary
Coordinate care for assigned patients and families by completing comprehensive physical and psychosocial assessments to identify intervention needs. Ensure appropriate specialist referrals and support adherence to treatment plans, including physician visits. Educate patients on disease processes and treatment options to facilitate informed decision-making while providing emotional counseling to improve transitions and adherence. Empower patients through goal-setting and advance care planning discussions. Partner with clinicians to manage care transitions between hospital, primary, and specialty settings, facilitating proactive discharge planning to reduce readmissions. Build relationships with the healthcare team while protecting patient privacy and confidentiality.
Required Qualifications
- Minimum 2 years' experience as Registered Nurse
- Current and Valid Registered Nurse License in the state position is based
- BLS certification required
Desired Qualifications
- 1–2 years of nursing experience in a community-based setting, such as home health, rehabilitation, hospice, or palliative care, preferred
- Knowledge of the interdisciplinary team concept
- Experience in patient education, planning, and management desired
- Exceptional communication and customer service skills
- Reliable transportation, current state driver's license and automobile insurance
- Bilingual a plus
- Bachelor's degree preferred
