Palliative Care Registered Nurse (RN)

Job Summary
Coordinate care for assigned patients and provide expert, individualized support to patients and families. Complete initial and on-going comprehensive physical and psychosocial assessments to identify needs and areas for intervention. Ensure appropriate referrals to specialists, support adherence to treatment plans, and assist patients in accessing community services. Educate patients and families on disease processes and treatment options to ensure informed decision-making. Provide emotional support and counseling to improve transitions and adherence to care plans. Evaluate recovery after treatment, encourage goal-setting, and facilitate advance care planning discussions. Build relationships with clinicians, manage care transitions between settings, and facilitate proactive discharge planning. Protect patient privacy and confidentiality. Minimum 2 years' RN experience required with 1–2 years in community-based palliative care preferred. Bachelor's degree and current state license required; BLS certification mandatory.
Required Qualifications
- Minimum 2 years' experience as Registered Nurse
- Current and Valid Registered Nurse License in the state position is based
- BLS certification required
Desired Qualifications
- 1–2 years of nursing experience in a community-based setting, such as home health, rehabilitation, hospice, or palliative care, preferred
- Knowledge of the interdisciplinary team concept
- Experience in patient education, planning, and management desired
- Exceptional communication and customer service skills
- Reliable transportation, current state driver's license and automobile insurance
- Bilingual a plus
- Bachelor's degree preferred
