Palliative Care Registered Nurse (RN)

Coordinate care for assigned patients and families by completing comprehensive physical and psychosocial assessments to identify intervention needs. Provide expert support through education on disease processes, treatment options, and advance care planning while facilitating emotional counseling to improve transitions and adherence. Ensure appropriate referrals to specialists and manage transitions of care between hospital, primary, and specialty settings with the interdisciplinary team. Assist patients in accessing community resources to enhance overall health and well-being, while protecting patient privacy and confidentiality. Promote a patient-centered approach to care and proactive discharge planning to decrease length of stay and readmission rates.
