Palliative Care Registered Nurse (RN)

Job Summary
Coordinate care for assigned patients and provide expert, individualized support to patients and families. Complete initial and ongoing comprehensive physical and psychosocial assessments to identify needs and areas for intervention. Ensure appropriate referrals to specialists, support adherence to treatment plans including physician visits, and assist patients in accessing community resources. Educate patients and families on disease processes and treatment options to ensure informed decision-making. Provide emotional support and counseling to improve transitions and adherence to plans. Evaluate recovery after treatment, empower patients through goal-setting and advance care planning discussions, and partner with clinicians to manage care transitions between hospital, primary, and specialty settings. Facilitate proactive discharge planning to promote better outcomes and reduced readmissions. Protect patient privacy and confidentiality. Minimum 2 years' RN experience required with 1–2 years in community-based palliative care preferred. Bachelor's degree preferred with current state license and BLS certification.
Required Qualifications
- Minimum 2 years' experience as Registered Nurse
- Current and Valid Registered Nurse License in the state position is based
- BLS certification required
- Reliable transportation

Desired Qualifications
- 1–2 years of nursing experience in a community-based setting, such as home health, rehabilitation, hospice, or palliative care
- Knowledge of the interdisciplinary team concept
- Experience in patient education, planning, and management
- Exceptional communication and customer service skills
- Bachelor's degree preferred
- Bilingual a plus
