Palliative Care Registered Nurse (RN)

Coordinate care for assigned patients and provide expert, individualized support to patients and families. Complete initial and ongoing comprehensive physical and psychosocial assessments to identify needs and areas for intervention. Ensure appropriate referrals to specialists, support adherence to treatment plans, and assist patients in accessing community services. Educate patients and families on disease processes and treatment options to ensure informed decision-making. Provide emotional support and counseling to improve transitions and adherence to care plans. Evaluate physical and emotional recovery, empower patients through goal-setting and advance care planning discussions, and partner with clinicians to manage care transitions between hospital and primary settings. Facilitate proactive discharge planning to promote better outcomes. Protect patient privacy and confidentiality. Requires current Registered Nurse license, BLS certification, and minimum two years of nursing experience. Bachelor's degree preferred.
