Palliative Care Registered Nurse (RN)

Job Summary
Coordinate care for assigned patients and provide expert, individualized support to patients and families residing in the community. Complete initial and ongoing comprehensive physical and psychosocial assessments to identify needs and areas for intervention. Ensure appropriate referrals to specialists, support adherence to treatment plans, and assist patients in accessing community services to improve health and well-being. Educate patients and families on disease processes and treatment options to ensure informed decision-making. Provide emotional support and counseling to improve transitions and adherence to care plans. Build relationships with clinicians, manage transitions between hospital and specialty care, and facilitate proactive discharge planning. Protect patient privacy and confidentiality when releasing information. Minimum 2 years' RN experience required, with 1–2 years in a community-based setting preferred. Bachelor's degree preferred; BLS certification required.
Required Qualifications
- Minimum 2 years' experience as Registered Nurse
- Current and Valid Registered Nurse License in the state position is based
- BLS certification required
- Reliable transportation

Desired Qualifications
- 1–2 years of nursing experience in a community-based setting, such as home health, rehabilitation, hospice, or palliative care
- Knowledge of the interdisciplinary team concept
- Experience in patient education, planning, and management
- Exceptional communication and customer service skills
- Bachelor's degree preferred
- Bilingual a plus
