Palliative Care Registered Nurse (RN)

Coordinate care for assigned patients and provide expert, individualized support to patients and families residing in the community. Complete initial and ongoing comprehensive physical and psychosocial assessments to identify needs and areas for intervention. Ensure appropriate referrals to specialists, support adherence to treatment plans including physician visits, and assist patients in accessing community services and resources. Educate patients and families on disease processes and treatment options to ensure informed decision-making while providing emotional support and counseling. Evaluate physical and emotional recovery, empower patients through goal-setting and advance care planning discussions, and partner with clinicians to manage care transitions between hospital, primary, and specialty settings. Facilitate proactive discharge planning to promote better outcomes and reduced readmission rates. Maintain patient privacy and confidentiality when releasing information.
