Palliative Care Registered Nurse (RN)

Coordinate care for assigned patients and families by completing comprehensive physical and psychosocial assessments to identify intervention needs. Ensure appropriate specialist referrals, support adherence to treatment plans, and assist in accessing community resources to improve overall health. Educate patients on disease processes and treatment options to facilitate informed decision-making while providing emotional support and counseling. Evaluate physical and emotional recovery, empower patients through goal-setting and advance care planning discussions, and partner with clinicians to manage transitions between hospital, primary, and specialty care settings. Facilitate proactive discharge planning to promote better outcomes and reduced readmission rates. Maintain patient privacy and confidentiality when releasing information.
