Job Summary
Conduct palliative care for patients with life-limiting diseases in home settings, managing symptoms, advance directives, and end-of-life decisions while collaborating with multidisciplinary teams including social workers and therapists. Assess psychosocial, physical, and spiritual needs of patients and families through direct home visits, developing relationships and answering questions as part of a collaborative care model. Schedule includes Monday through Friday, 8am-5pm, with options for 4 or 5 days per week and a weekend rotation approximately every 4 weeks. Compensation is visit-based, ranging from $54.00 to $104.00 per visit depending on visit type and experience.
Required Qualifications
- Current California RN License
- At least one year of professional nursing experience
- CPR certified (Healthcare Provider)
- Reliable vehicle to drive for work
- Current Driver's license and clean driving record
- Have and maintain auto insurance coverage
- Comfortable working independently in a home-visit setting
- Must be able to effectively communicate with elderly and chronically ill patients and families
- Regular work schedule is Monday through Friday, 8am-5pm with a choice between 4 or 5 days/week and a weekend rotation approximately every 4 weeks
Desired Qualifications
- Home Health and/or Hospice experience is a plus
