Job Summary
Nurse Practitioner sought to join Tufts Medicine Care at Home for home-based palliative care in the greater Lowell/Malden area, with occasional virtual hospice coverage. Responsibilities include applying clinical standards in hospice/palliative care, providing medical expertise on pain and symptom management, discussing goals of care with patients, evaluating issues and generating alternatives, maintaining documentation of patient visits, and collaborating within an interdisciplinary team. Requires MA RN and NP licenses, board-certified in hospice/palliative care, and a collaborative, team-oriented approach.
Required Qualifications
- Current license as a Registered Nurse and Nurse Practitioner in the Commonwealth of Massachusetts
- Graduate of an accredited and/or approved nurse practitioner program
- Extensive knowledge, experience and expertise in hospice and palliative care
- Interpersonal skills to establish and maintain relationships within the medical community
- Demonstrated ability to work as a team player and function as an interdisciplinary team
