Job Summary

Conduct routine and emergency assessments for patients in palliative and hospice care, including admissions, transfers, and recertification visits. Perform bedside evaluations, manage acute and chronic conditions, and prescribe medications within scope. Order and interpret laboratory and radiology tests, order durable medical equipment, and coordinate care with the interdisciplinary team and physicians. Complete clinical documentation within 24 hours of visits and initiate reimbursement for services rendered. Provide education on disease processes, symptom control, and advance care planning to patients and families. Maintain accurate patient records and collaborate on care coordination and clinical practice guidelines.

Required Qualifications

Master's degree in Nursing
Active and unencumbered CNS or Nurse Practitioner license in state of employment
Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric
Active NPI number
Active DEA certification if state allows prescription of controlled substances by APPs
Current collaborative physician agreement in place
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
Ability to write reports, business correspondence, and procedure manuals
Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
Strong written and verbal communications
Ability to understand, read, write, and speak English
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
Ability to compute rate, ratio, and percentage
Ability to lift and move objects up to 50 pounds
Close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
Must be able to work a flexible schedule to include nights and weekends
Must be able to travel
Ability to maintain confidentiality of information, such as patient and Company files
Minimum of one (1) year nursing practice or equivalent experience
Minimum of five (5) years nursing experience
Clinical nursing care in oncology, hospice, geriatrics, intensive care
Ability to triage consults by patient acuity

Ability to complete and submit required paper work within 24 hours of visit completion
Ability to complete and submit required clinical documentation within 24 hours of visit completion
