Job Summary
Palliative Care Nurse Practitioner opportunity in Concord, NH for a Per Diem role with Impact Health PC. Deliver person-centered, autonomous care in Skilled Nursing Facilities with physician-level consult support. Responsibilities include conducting assessments, developing treatment plans, managing follow-up care, educating patients and families, completing timely documentation (within 24 hours), and ensuring regulatory compliance. Requires an active NP license, 2+ years post-certification experience, and experience in SNFs, hospitalist, ED, or ICU settings; proficiency with EHRs; strong collaboration skills. Required Qualifications
- Active license as a Nurse Practitioner (NP)
