Job Summary
Deliver person-centered palliative care within Skilled Nursing Facilities (SNFs) with autonomous care under physician-level consults. Responsibilities include conducting assessments, developing treatment plans, managing follow-up care, educating patients, families, and staff, and completing timely documentation within 24 hours while ensuring compliance with facility, state, and federal protocols. Requires an active NP license and at least 2 years post-certification experience, with preferred experience in SNFs, hospitalist roles, ED, or ICU; proficiency with EHRs and clinical technology; strong communication and collaboration.
