Job Summary
Palliative Care Nurse Practitioner needed for a per diem (PRN) role in Bucks County, PA. Responsible for delivering person-centered palliative care in Skilled Nursing Facilities, collaborating with interdisciplinary facility teams, conducting assessments, developing treatment plans, and managing follow-up care. Must perform chronic care management, educate patients, families, and staff, and complete documentation within 24 hours. Role includes autonomous practice with physician-level consult access and ongoing clinical support. Qualifications include an active Nurse Practitioner license and a minimum of 2 years post-certification experience; SNF, hospitalist, ED, or ICU experience preferred. Proficiency with EHRs and strong collaborative communication skills required. Compensation ranges from $115,000 to $200,000 annually (salary-based with bonuses), plus generous benefits including PTO, 401(k), comprehensive health benefits, malpractice insurance, license reimbursement, CEU support, paid training and onboarding.
