Job Summary
Conduct assessments, develop treatment plans, and manage follow-up care for patients in Skilled Nursing Facilities using evidence-based, person-centered practices. Collaborate with interdisciplinary facility teams to provide ongoing chronic care management while educating patients, families, and staff. Complete timely documentation within 24 hours and ensure compliance with facility, state, and federal regulations. Deliver independent care with access to physician-level consults. Required Qualifications
- Active license as a Nurse Practitioner (NP)
