Job Summary
The Palliative Care Nurse Practitioner provides direct patient care within Pallitus Health Partners’ palliative care program, conducting medically necessary palliative visits and hospice recertification, and rotating to cover general on-call duties for orders and prescriptions. The NP collaborates with the Interdisciplinary Group (IDG), participates in education, outreach and leadership activities for the community location, and may take on additional patient-care or administrative duties as assigned. Requires MSN or higher, licensure in KY and IN with DEA/CS permits, at least one year of advanced practice experience (preferred in primary, palliative, hospital-based, or hospice settings), and a valid driver’s license. Proficiency with electronic medical records and willingness to fulfill on-call duties are also expected. Schedule is Monday–Friday; location is Louisville, KY with in-person duties; salary is paid as a salary position.
Required Qualifications
- MSN or higher-level Adult Nurse Practitioner
- Licensure (or eligibility) for advanced nursing practice in both KY and IN, including DEA and controlled substance permits
- Minimum of one year experience in full time advanced nursing practice preferred
- Primary care, palliative care, hospital-based, or hospice experience preferred
- Fulfill requirements of on-call duty policies
- Valid driver’s license
- Use of technology-based tools, including electronic medical record, strongly preferred
