Palliative Care Nurse Practitioner

Job Summary

Conduct routine and urgent assessments for patients with chronic, complex, or life-threatening conditions, performing bedside palliative care, disease education, and symptom management. Order, interpret, and prescribe medications and treatments while managing a minimum census load of 80 patients. Collaborate with the Medical Director, primary physicians, and interdisciplinary teams to coordinate referrals, define treatment goals, and communicate psycho-social interventions. Complete accurate clinical documentation within 24 hours of visits, manage billing and coding per CMS guidelines, and provide onboarding support for trainees. Work Monday through Friday with after-hours and weekend on-call availability, including regional travel within a 60-mile radius.

Required Qualifications

  • Master's degree in Nursing
  • Minimum of five (5) years nursing experience
  • clinical nursing care in oncology, hospice, geriatrics, intensive care
  • Active and unencumbered CNS or Nurse Practitioner license in state of employment
  • Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric
  • Active NPI number
  • Maintain active DEA certification if state allows prescription of controlled substances by APPs
  • Current collaborative physician agreement in place
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
  • Ability to write reports, business correspondence, and procedure manuals
  • Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
  • Strong written and verbal communications
  • Ability to understand, read, write, and speak English
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
  • Ability to compute rate, ratio, and percentage
  • Ability to lift and move objects up to 50 pounds
  • Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
  • Ability to work a flexible schedule to include nights and weekends
  • Ability to travel
  • Ability to maintain confidentiality of information, such as patient and Company files

Desired Qualifications

  • Board Certification in Hospice and Palliative Care
