Palliative Care Coord

Job Summary
Coordinate palliative care program growth and development by leading, educating, and mentoring providers within the organization. Maintain up-to-date knowledge of regulatory changes to support goal planning and education for agency staff and community partners through workshops and bedside consultations. Collaborate with the palliative care team on care interventions, review practice protocols, and drive quality performance improvement projects including MIPS tracking. Provide direct patient care by maintaining a regular panel, conducting face-to-face hospice assessments for eligibility determination, and arranging meetings with prospective patients and families. Complete accurate clinical notes documenting pain management and outcomes within 24 hours, while resolving interpersonal and ethical concerns. Advance hospice and palliative care through involvement with local, state, and national trade organizations.
Required Qualifications
- Minimum of five (5) years of professional experience in health care
- Experience in a hospice care
- Experience in palliative care
- Experience in palliative care, preferably 2+ years
- A master's or doctoral degree in nursing or physician assistant studies
- Current CPR Certification
- Current Massachusetts licensure: Nurse Practitioner or Physician Assistant
- Valid driver's license
- Hospice and Palliative Care Nurse Certification
- Experience in operational care delivery coordination
- Experience working with an Interdisciplinary Team
Desired Qualifications
- Experience in homecare
- Understands hospice and palliative care philosophy, and issues of death/dying, goal concordant prescribing and serious illness conversations and goals of care establishment
- Demonstrates good verbal and written communication, and organization skills
- Strong observation skills and problem-solving skills
