Job Summary
Deliver high quality palliative care to patients with medical necessity via in-person and telehealth options. Assist with design of operational processes for the palliative care program and work with facilities to ensure its growth. Train and educate internal employees on palliative care principles and processes. Recognize gaps in current operations and collaborate with interdepartmental team members to streamline and organize palliative services. This role supports Eventus WholeHealth's integrated model of care for adults in skilled nursing and assisted living facilities.
Required Qualifications
- Masters degree in Nursing from an accredited educational institution
- current and valid license to practice medicine issued by the state in which you reside Medical Board
- All other certification necessary to perform one's duties must be current and valid
- Have graduated from a physician assistant educational program that is accredited by a National Commission on Accreditation of Allied Education Programs
- Have passed the national certification examination of the National Commission on Accreditation of Certification of Physician Assistants
- Licensed by the state in which you reside as a Physician Assistant
