Job Summary
Oversight and care of patients in Palliative Care programs including Transitions, Living with Cancer, and Journeys for the Grand Valley. Develop and implement individualized patient care plans for those experiencing serious illness, coordinating care through the healthcare system while focusing on comfort and symptom management. Discuss goals of care and assist with advance directives to navigate their care, ensuring timely transition to hospice when eligible. Maintain a weekday schedule with occasional on-call or weekend requirements in Grand Junction, Colorado.
Required Qualifications
- Current Colorado RN license maintained throughout employment
- Current CPR certification maintained throughout employment
- Valid Colorado driver's license
- Reliable transportation
- Proof of insurance
Desired Qualifications
- Minimum 1–2 years of nursing experience preferred
- Experience in home health, palliative care, or acute care strongly preferred
- Strong communication and interpersonal skills
- Ability to navigate emotional or high-stress situations with professionalism and compassion
- Experience with EHR systems
- Experience with Microsoft Office applications
- Bilingual fluency
