Job Summary
Serve as the primary point of contact for patients and families enrolled in the palliative care program, providing emotional and logistical support to help them make informed decisions. See patients onsite when needed to assess conditions, review care plans, and offer immediate assistance. Facilitate timely communication between the healthcare team, patients, and families while coordinating resources to optimize care. Educate caregivers on medication, symptom management, and available services, including advanced care planning discussions. Track detailed patient records, participate in team meetings, and collaborate with external agencies to ensure continuity of care. This part-time, field-based role in the San Francisco Bay area supports physicians by reducing administrative burdens so they can focus on patient care.
Required Qualifications
- Current LVN license in the state of California
- Minimum of 2 years of clinical experience
- Experience in palliative or hospice care
